    An annual diabetes cycle of care review is essential to optimise management and prevent complications for people with diabetes. Despite this, between 30-70% of patients with diabetes are not receiving investigations for key cycle of care components that could optimise their care.¹

    Join our type 2 diabetes (T2D) experts who also have a special interest in primary practice quality improvement. They'll provide valuable advice for goal setting with patients and the wider team, tips for optimising your practice systems, implementation of quality care plans and education about use of MBS item numbers – with the T2D patient at the centre of care.

    Why take part in this program?

    • Learn about managing T2D targets and comorbidities, and how your practice can work collaboratively to assist with the implementation of these T2D cycle of care targets.
    • Earn 21.5 CPD hours upon completion.
    • Your practice staff (such as your practice manager, practice nurse, receptionist) that you engage to work with you on this program can also be provided with quality improvement completion certificates.

    Open to GPs, practice managers, practice nurses and other relevant practice staff. Gain 21.5 CPD hours and a quality improvement completion certificate.

    Learning outcomes

    At the end of this program you will be able to:

    • Outline the benefits of an annual diabetes cycle of care to help prevent complications in patients with T2D
    • Assess current measurements for selected diabetes cycle of care targets in your patients, for review and quality improvement
    • Using a team-based approach, develop multimodal quality improvement goals related to the annual cycle of care for your patients with T2D
    • Implement and reflect on quality improvement initiatives, practice-wide.
