Activity aim
In this clinical audit, you will review the management of patients with CKD in line with latest Australian guidelines, utilising the 'kidney health check' to identify the condition in at-risk patients as appropriate. You will stage patients' CKD severity in order to guide their management, and develop comprehensive management plans involving lifestyle, diet and pharmacological management strategies.

- Earn 6.5 CPD hours upon completion.
Inclusion criteria
Patients who are eligible for this clinical audit are adults who are:
- aged 18 years or older
- have an existing diagnosis of CKD with recent eGFR and uACR measurements
OR
a new diagnosis of CKD identified using a 'kidney health check' - not currently being managed by a specialist for their CKD.
How long should it take?
The program has been designed to take approximately 4-6 weeks to complete. This includes patient recall from your practice, patient consult and program evaluation.
Tips to recruit patients
Recruitment may occur:
- Opportunistically as you consult appropriate patients as part of usual care.
- By active recall.
Learning outcomes
On completion of this educational activity participants will be able to:
Assess patients who may be at risk of chronic kidney disease using a Kidney Health Check.
Apply evidence-based tools to stage CKD severity in order to guide management.
Evaluate the cause of CKD and any contributing comorbidities.
Develop a holistic management plan to manage renal function and quality of life.
Identify factors that indicate referral to a nephrologist.
